Thats because by default the usage of H key is/was blocked in Milfy (is it still?). Use unren to allow back standard Ren'Py fuctions like the H key. But that may not help. There was a post on this quite a while ago, need to threadsearch it.

This MC never seek the hapiness from sarah. He only hurt sarah on roof. Sarah say him many time, this hurt. But he never hear on Sarah. But, in oposit to this it give in Milfy City also lovely scenes . Like this:

So i installed the incest patch but it doesn't seem to work. I'm still on the beginning but it looks like he isn't related to his "family". That makes me sad :(
